How we are building a bridge between fragmented care and connected well-being
When a patient has a complex, multi-morbid condition, the problem is rarely a lack of medical knowledge. The problem is disconnection.
The local clinician has part of the picture. The specialist has another part. The patient carries the rest in their own memory, in paper files, in messages that never reach the right person. Knowledge exists - but it does not flow.
Bridgescape's Integrated Complex Care pilot is our answer to that fragmentation. It is not a product. It is a bridge. And it is being designed around five principles.
In most systems, the record belongs to the institution. It sits in a hospital database, a clinic file, a specialist's notes. When the patient moves, the record does not move with them.
We are designing for the opposite: a patient-controlled data layer where the patient owns their health record, decides who can see it, and carries it across providers, clinics, and borders. This is not a technical preference. It is a principle: the person at the centre of care should hold the story of their own health.
Today, when a local clinician needs a specialist's opinion, the process often relies on informal messages, phone calls, or paper referrals. Information is lost. Time is wasted. The patient waits.
We are designing a structured referral and co-management pathway - a defined format for sharing the patient's history, symptoms, and context with an external specialist, and for receiving recommendations back. No more repeating the story. No more chasing.
A complex case is never only medical. Nutrition, physical activity, mental health, and the patient's living environment all shape outcomes. Yet these dimensions are rarely part of the clinical record.
We are designing for holistic care planning - where the care plan can include nutrition, movement, mental resilience, and an optional Environmental & Living Conditions checklist covering housing, green space, air quality, and contact with animals. Because human health cannot be separated from the wider web of life.
Any tool that adds time to a clinician's day will fail. This is non-negotiable.
We are designing for low burden - so the platform reduces administrative work rather than adding to it. The referral is faster. The record is already there. The specialist's response is structured. The clinician spends less time on paperwork and more time with patients.
Health data is among the most sensitive data that exists. We are designing with privacy by design - patient consent at every step, encrypted data, no sharing without explicit permission, and full transparency about who has access and why.
The Integrated Complex Care pilot is currently in its research and partner recruitment phase. We have completed in-depth interviews with physicians in Montenegro, and we are preparing the patient research phase, including ethics approval. Co-design is now underway with private clinics and clinicians taking part in the research, and they are helping shape how the bridge works.
We are not deploying technology yet. We are building the evidence, the relationships, and the design principles first. Because a bridge that is not built with the people who will cross it is not a bridge at all.
This is the bridge we are building. Slowly, carefully, and with the people it is meant to serve.
